Tibial neurostimulator device treats urge urinary incontinence

The Capital Region’s first Altaviva device, a tibial neurostimulator for the treatment of urge urinary incontinence, was implanted at Albany Medical Center in August by Lara MacLachlan, MD, Falk Endowed Chair and professor in the Department of Urology.

“This is an exciting addition to the treatments available for incontinence and marks another milestone in our ongoing commitment to offering patients the most current and effective treatment options available,” said Dr. MacLachlan.

Urge urinary incontinence affects both men and women and occurs when the sudden urge to urinate cannot be controlled. It differs from stress incontinence, which is caused by actions like sneezing or laughing, and overflow incontinence, which occurs when the bladder isn’t able to empty completely.

Approved by the FDA in September 2025, Altaviva is implanted just under the skin on the inside of the ankle near the tibial nerve. This nerve runs from the spine down the leg and is connected to nerve pathways in the spine that control the bladder. “When the device is activated, it sends signals through the tibial nerve to the bladder to calm down,” explained Dr. MacLachlan.

The minimally invasive procedure to implant Altaviva is quick (about 15 minutes). It takes place under local anesthesia in an operating room for sterility purposes and doesn’t require general anesthesia or sedation. The battery needs to be charged wirelessly one to two times per year and will last approximately 12 to 15 years.

Currently available through the urology clinic at Albany Medical Center, Altaviva will also be offered by OBGYN providers at Albany Medical Center, and at Albany Med Health System hospitals in Glens Falls and Saratoga Springs.
