New Patient Portal: Sign Up Now!

To make your connection to the Albany Med Health System even stronger, all System hospitals are switching to a new patient portal, MyChart. Albany Medical Center is now live on MyChart. Columbia Memorial Health, Glens Falls Hospital, and Saratoga Hospital will be live on Nov. 2, 2024.

All patients of all of our hospitals can sign up now ahead of the Nov. 2 live date, and should register for a MyChart account. Please see the FAQs below for more information about the transition and how to download your health records if you have a current patient portal account at one of our hospitals.

If you have already created a MyChart account as an Albany Medical Center patient, you do not need to sign up again.

About MyChart

The Albany Med Health System is transitioning to a single, unified electronic medical record, which connects patient records for inpatient and outpatient services, scheduling, registration, and billing throughout each System hospital and clinic location. That means no matter where you seek care in the health system, your information will be readily available to all our clinical teams. MyChart will be our new secure 24/7 online patient portal that allows you to easily manage your own health care, including refilling your prescriptions, managing your appointments, messaging your doctor, and more.

No, there is an option to sign up without an activation code. You can sign up now to skip the activation code.

Until Nov. 2, 2024, you should continue to use the eClinicalWorks Healow portal to view test results. On Nov. 2, patients will no longer have access to Healow. Portal users should create a new account in MyChart for a seamless experience.

To download your records from Healow, follow these steps before Nov. 2. If you are using a web browser, log in to your Healow account. From the dashboard, click “Request PHR” in the Medical Records section. If you are using the Healow mobile app, log in to your account and click “My Records.” Enter the date range desired then click submit. Once the record has been generated, which may take several minutes, click on the desired file type to save your records.

You can also request your records from Columbia Memorial Health.

For patients who use My Health Record, the self-scheduling feature will be deactivated on Oct. 18, 2024, and the messaging feature will be deactivated on Oct. 29, 2024. On Nov. 2, patients will no longer have access to My Health Record. Portal users should create a new account in MyChart for a seamless experience.

To download your records, log in to your My Health Record account before Nov. 2. Once logged in, click on Health Record in the upper left of the screen. From here you can click on any of the icons to view, print, transmit, or download any of the information in your electronic medical record. For results, visit summaries, clinical notes and microbiology, then specify a date range for the results you wish to view.

You can also request your records from Glens Falls Hospital.

On Nov. 2, patients will no longer have access to the Medent patient portal. Portal users should create a new account in MyChart for a seamless experience.

To download your records, log in to the Medent patient portal before Nov. 2, 2024:

Click on “Chart” and then “Chart Summary.” From there, you can select which information you would like to download and the date range or select “entire history” from the dropdown menu and “Select All” to download all your records from Medent. Then, select either “Download” or “Email” to have your records sent to your email.

You can also request your records from Saratoga Hospital.
