Community Health Needs Assessment and Implementation Strategy

Albany Medical Center is a valuable asset and resource for the many communities it serves. A member of the Albany Med Health System, it is also the only academic health sciences center in the 25 counties of eastern New York and western New England. It includes the 766-bed Albany Medical Center, founded in 1849 and one of upstate New York's largest teaching hospitals; the Albany Medical College, established in 1839 and one of the nation's first private medical schools; Albany Medical Center's South Clinical Campus, purchased in 1999 to better serve the ambulatory surgical needs of the community; and the 500-member Physicians Group.

An academic medical center like Albany Medical Center is defined by its commitment to the three-fold mission of excellence in patient care, biomedical research and medical education - with a single objective: the improved health of our community. This objective is measured in our Community Health Needs Assessment and Implementation Strategy.

Because of our unwavering commitment, assessing the health care needs of our community is an ongoing process. We are engaged in countless affiliations and collaborations throughout our service area and offer many programs and services at a free or reduced-fee basis, often in partnership with other organizations.

In 2022, a comprehensive Community Health Needs Assessment and Implementation Strategy was conducted with partner organizations, under the direction and guidance of the Healthy Capital District Initiative (HCDI). The results of this in-depth study serve as the platform for our 2022-2024 Community Service Plan, also referred to as our Health Improvement Plan.

The Community Service Plan focuses on several significant health priorities within our local community, and identifies how Albany Medical Center - with partner organizations - is working to:

  • Execute a community health improvement plan;
  • Reduce duplication of services and costs;
  • Assist each other for improved efficiency and efficacy; and
  • Maximize available resources and assets
