Albany Med Health System Adds Well-Known Family Medicine Practice in Slingerlands

Delmar Family Medicine has joined the Albany Med Health System, giving the practice’s patients seamless access to advanced care at Albany Medical Center, the region’s only academic medical center.

Delmar Family Medicine, located at 1499 New Scotland Road in Slingerlands, has provided primary care to families in the Bethlehem area for nearly 20 years. Led by Peter Forman, MD, and Rebecca Elliott, MD, the practice is also home to Carly Canavan, PA, and Nanette Gamon, NP. The practice sees patients of all ages.

The office will remain at its current location, inside the former Slingerlands Methodist Church, in the Slingerlands Historic District in Bethlehem.

“We will continue to provide our patients with high-quality preventative care for the whole family. Now, as part of an academic health system, we offer patients access to the most up-to-date clinical research and best practices, along with direct referrals to the region’s largest and most advanced team of specialists,” said Dr. Forman, who founded the practice in 2007 and moved to the Slingerlands location in 2016. Dr. Forman completed a residency in family medicine at Albany Medical College in 2003 and is medical director of the College’s Physician Assistant Studies Program.

The Albany Med Health System is home to more than 30 primary care offices throughout the Capital Region from Columbia to Albany to Warren counties. Internal medicine and family care physicians across the System provide patient-centered primary care including direct referrals to specialists, age-specific screenings, and coordination of hospital care when needed.
