Offering Patient-Centered Primary Care

We are accepting new patients. Our providers offer routine checkups, immunizations, same-day sick visits, chronic condition management, behavioral health, health care screenings, and community education.

• We are your partners in care. We work with you to manage your health and make sure decisions about your care reflect your needs, wants, and preferences. We also provide education and support so you can be an active participant in your health care.
• We take a team-based approach so we can meet most of your health care needs throughout your lifetime. Our team members provide prevention and wellness services, acute and chronic care, rehabilitation, and behavioral health care.
• We coordinate your care with providers and organizations across the health care system, from specialists to hospitals and rehabilitation centers to home health care and community support service providers.
• We make it as easy as possible to get care and information when you need them. We deliver urgent care with shorter waiting times and we have extended hours for certain services. We also offer 24/7 email or phone access.
• Our providers and staff share an overriding commitment to quality and safety. We practice evidence-based medicine, care deeply about patient outcomes and satisfaction, and constantly seek new ways to enhance your patient experience.
